cool hospital day!!

a girl let me help her do a complex dressing today.

her patient had just undergone a fasciotomy.

IT LOOKED SO COOL!

*warning graphic for some!*

kinda like this http://www.trauma.org/resus/teamlead.../scene024a.jpg

except this patient's bone was visible. the patient even told me to take pictures of it.

one of the coolest things I've seen outside the operating room so far!

I don't know how "cool" that would look, but I'm glad somebody wants to do that kinda stuff, cuz i don't.

what is a fasciotomy anyway, and why does it look like there's a bottle in that person's leg/arm?

a fasciotomy is the removal of the fascia, a type of connective tissue that's around muscles and other organs and helps to support them.

after a traumatic injury, a lot of inflammation, pressure/tension can build up within the fascia and so the removal of it allows for it to prevent further tissue death.

and the "bottle" inside the leg is actually a muscle.

i see it now, i didn't really look that closely the first time and the stuff over the outside of the muscle makes them gleam a little.

What happened to the person's leg - were they in car crash?

Why wouldn't they stitch up that giant open incision right after the operation?

I don't know what happened to that person's leg in the picture, that was only to demonstrate what a fasciotomy may look like.

I actually don't know all that much about fasciotomies, as it wasn't my patient. but from what I know, the patient who I saw has some sort of accident, involving a huge piece of glass shattering into her arm. she almost bled to death, but was lucky because someone wrapped a belt and a piece of cloth around her arm right on time to prevent this from happening.

and they don't stitch it up because you want the connective tissue to grow back into that open space, thus making the fascia longer.

could you see her soul anywhere in there? Or is that only during heart surgery... or is it the brain now?

I was watching a lung transplant on "The Operation", it looked really weird when they had the person's ribcage almost completely removed. You should try and get on that show Joanne.

I know! stuff like that is so neat to watch... I was watching this show yesterday I don't think it was life the ER but it was on ch.34 (TLC) .. some guy was stabbed near his heart so they had to cut open his chest all the way to his back and open him up and massage his heart. that was one of the coolest things I have ever seen!

the problem with those reality tv shows is it fails to depict how mundane the whole surgical process can be at times.

I've seen maybe 6 types of surgeries but I've never been interested in staying for the whole entire procedure because it gets really boring.

most of the surgery goes like this:

"ZZZZZ" (as the the surgeon proceeds to cauterize. he does this for about 1 second)

*wipes the area down the area*

"ZZZZZ.... ZZZZ" (as the the surgeon proceeds to cauterize. he does this for about another second)

then, depending on what the process is, repeat this for about another 1-2 hours until the surgeon finally gets down to the area that needs manipulation or removal.

yadayadayada.

don't get me wrong, I find surgeries interesting.. but after a while.. it's just the same old thing being repeated for an hour or so until something neat finally happens. mind you, I've never physically seen an emergency surgical procedure, or anything involving a scope.. perhaps those might be more interesting..

but I guess I'm more of a blood and gore kind of gal.
