stupid dental claims

as everyone knows im fixing up my fooked up toof.

I went through my dentist to get my forms in.
they wouldnt accept it
I went through the website, it says my employee number is wrong.
I called and they said my employee number is wrong.

I have a really expensive appointment on the first if i dont get this shit done.

Hopefully the HR guy can help my resolve this issue.
