Sometimes ill tip alone just becuase the food was soo good, or ill not want to tip becuase I thought it was terible. From what I understand (depending were you work) the kitchen usualy gets some kind of tip out. I think its a pretty fair game, but deffinently relise on team work.

Personaly I dont like getting alot of service in ways of being pampered or me being lazy. Im one of those ppl who likes to help the waitress clean up the table lol. Or if I feel ive hassled the wiatter/weaitress's job, left a huge mess, or been rather obnoxiouse ill deffinently leave some extra.
As for good service... I dont really notice it untill I get bad service. I mean theres the average nice and friendly service, does the job right etc.
And then theres GREAT service for example; someone doing you a favour, fix up your meal just right, remembers your favorite everytime, a little extra tlc in a drink (extra booze) or no charge to sober drinks, instead of telling me "sorry were out" goes and at least trys to find it in the back or ask the manager. When I get an HONEST respose/sugestion instead of tyring to up sell. I know alot of servers try to 'chit chat' and it seems kinda fake, but then somtimes you actualy get in a real type of conversation and id think of that as really good freindly service. Some places just have a really good all round vibe... therefore it imediatly puts me in a good mood and ill feel like giving more ;)
